Adapt plugin to design
Adds the necessary markup and styling (originally posted in torque!111 (closed)), along with adapted JavaScript, for leaving feedback.
Note that the dropdown currently always appears right-aligned by the feedback buttons. This is based on our existing use case of putting the feedback toward the right side of the page. If this isn't always the case, we'll need to make this configurable / dynamic. (That's a sensible improvement task.)

- torque!113 (merged) should be merged before this plugin is added to the Explore page.
