Show more information about who did what and when

Instead of having a single word that's configured as the action, take a
messageKey that then allows external users to have more control over the
language that shows up in the log files.

Also allow arbitrary extra parameters to then be added for use in that
message if needed.  This removes the need for the comment.

Add CONTRIBUTING.md as per issue #3

Move log entry out of the "comment" field. Callbacks must now return an
array with the user, action, page, and an optional comment instead of a
string.

In reference to issue #1

As specified in issue #1. Booleans and strings are also accepted, for
instances where just the hook name or a default message would suffice.

This was the code that used to reside at
github.com/OpenTechStrategies/torque but needed to be split into
its own repository for maintenance reasons.
