Do not run submission custom code in update forms save method to avoid unwanted side effects like long file names

* Add PAF model and link to funds and labs

* Integrate the extra fields for the PAF into the PAF form

* Create a project on accepted determination
* Only create projects when submission is in the final stage
* Wrap determination creation in a transaction
Various parts of setting a determination can fail so lets tie them all
together into a transaction and only send a message when they're all
done successfully.
