Feature/gh1364 paf custom fields (#1479) 

* Add PAF model and link to funds and labs

* Integrate the extra fields for the PAF into the PAF form

* Update tests to work with the new PAF field

Fixes some larger issues around how data is serialized and deserialized from the
db by the submissions class.
