Fixes #3290

Adds an organization name column to the submission table. To effect that
change, a new unique block needed to be added. I did test that the
column will not show up when added to
`SUBSMISSION_TABLE_EXCLUDED_FIELDS`

Fixes #3452 

Current project views are that the docker setup isn't the best way to
roll out to prod. Adding that to the docs gives readers a better idea of
what to expect as an outcome from this section.
