From the ETL pipeline work on June 22, 2021

There are places where we want to specify what the name in the output
is, rather than letting it inherit.  At the same time, changed
LinkedSecondSheet to take a list of dictionaries, rather than multiple
arguments.

This required, for some of the competitions, to run the data through
dos2unix, which changed the shas.  But also, at the etl library level,
we needed to remove the unicode <feff> character that sometimes gets
added to the start of the csvs.

Also adds the export tables role to all the competitions.

This updates all competitions to use simplebook instead of collection.

If an existing deployment is using collection, the wiki configuration
must be manually updated (or explicitly replaced) so that both
simplebook and collection are not present at the same time

Issue #122

Now that globalview is fully deployed in production, it can be the prod
ansible target, holding port 5000, and the sha for that TDC extension
can be set to the globalview aware one.

This comes from an exterior sheet, and used for sorting.  Also included
a new CellProcessor for default values.

Requires some other foundational changes including:
* Adding an etl column for originating competition
* Updating the torque version to enable editing
* Updating TOCs to not include empty groups

This updates the modern competitions to use the common deploy tooling,
which includes a decrypt helper function and the help documentation that
explains how to call the script.

There are a few legacy competitions which have not been updated, but
potentially could be in future.

Also included a template change where Attachments was forgotten

Just copied over from the template and replaced all the variables.
