Add more documentation

The installation documentation right now is a bit sparse for the server components, and it is also spread across multiple directories.

Lets consolidate the installation instructions into the root README and expand them while we go. There may also be mental-mapping benefit to reorganizing the file structure so that we dont' have PHP => Python => Node nested directories, but that can be handled in a separate issue.
